Discharge Medicines Service

When you are discharged from hospital, our Discharge Medicines Service provides direct support from your pharmacy team. This service links you with our pharmacist, helping you understand your medicines and manage them safely at home.
Green Street Pharmacy – proudly supporting patients across Borden, Bapchild, Tunstall, Rodmersham, Milstead, Newington and the wider community.

What the Service Involves

Our pharmacist will review all the medicines you take and discuss:
  • Any new medicines started in hospital
  • Any changes made to your existing medicines
  • How and when to take each medicine correctly
They will explain what each medicine is for, common side effects to look out for, and how to use them safely. You’ll have the opportunity to ask questions and get clear, personalised guidance.

How This Service Helps You

Taking medicines correctly is essential for your recovery. Around half of all medicines are not taken as prescribed, often due to:
  • Taking the wrong dose
  • Taking medicines at the wrong time
  • Forgetting to take them altogether
Misuse of medicines can lead to your condition worsening or becoming harder to manage. By understanding your medicines and how to take them properly, you can improve your treatment outcomes and feel more confident managing your health at home.

Working Together for Your Care

Our pharmacist will support you throughout your recovery and, where needed, work closely with your GP to ensure continuity of care.
